Conference Registration

Registration Instructions

  1. Register for the conference using the form below.
  2. Reserve your hotel room(s) at the Embassy Suites Fort Lauderdale. (This step is separate from conference registration.)
  3. If you have received credits through Cincinnati Children's Hospital Medical Center in the past, if possible, please make sure that the email address you use on the conference registration form matches the email address used on the CME profile.
  4. Complete your fee payment (if fee is due) on the next page.

Complementary site registrations: Injury Free sites that have paid the annual membership fee may submit registration forms for as many as four attendees free of charge. Sites who have paid their dues receive payment voucher codes that must be entered below.

Hotel and Transportation

Hotel Reservations: You may make reservations online (preferred) or call Embassy Suites at 1.800.EMBASSY (1.800.362.2779), and use the group code IFC. You will need a credit card to hold the room. The credit card will not be charged until you check into the hotel.

Transportation from the Airport: A taxi from the airport to the hotel is about $13 1-way. This fare may be split if there is more than one person travelling in your group. If there are two or more people arriving in your party, you may call the hotel conierge at 954-315-1313 and arrange a hotel shuttle for $8.00.

Parking: If driving, the address of the hotel is 1100 SE 17th Street, Fort Lauderdale, Florida 33316. The hotel offers self-parking for $20/day (including in and out) and valet parking for $20/day.

Registration for CME Credits or Contact Hours

Injury Free has applied AMA PRA Category 1 Credit™. For more information, please see the Accreditation statement below.

If you desire to receive CME credits or contact hours for your participation in the conference, when registering for the conference, you MUST check the "Request CME certificate" checkbox below. Cincinnati Children's Hospital Medical Center will provide credit for the conference. You no longer need to pre-register in CCHMC's CME system. The e-mail address you use on the conference registration form will be used to send you information after the conference to claim credit. Note: BOTH types of credits will be processed through this system.

If you created a profile in CCHMC's system for last year's conference, if at all possible, please use the same email address when registering for this year's conference.

Accreditation Statement

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of Cincinnati Children's and The Injury Free Coalition for Kids. Cincinnati Children's is accredited by the ACCME to provide continuing medical education for physicians.

This activity has been approved for AMA PRA Category 1 Credit™.

Application for CHES (entry-level) / MCHES (advanced-level) Category I continuing education contact hours (CECH) has been made to the National Commission for Health Education Credentialing, Inc. (NCHEC).
